Why are there 2 ports on one end .which port is inlet & outlet

The two ports on the base end allow for alternate plumbing. Only one is used at a time on that end.

Is there a modification needed to switch from double action to single action and visa versa?

It isn’t recommended to use these cylinders in a single-acting application, but they could use a brass breather vent. It could possibly leak. This is a quick fix, but we would suggest a custom cylinder for better performance.
