What is the silver piece with 5 holes in it used for? Is that to extend the lifting height of the foot?

They are mounting brackets used for mounting the jack to the frame of the trailer.

When retracted, what is distance from bottom pad to top of mounting bracket?

According to our technicians the measurement from the bottom of the pad to the center of the bracket is 11", we don't have a measurement to the top, but it should be within about 2-3" of that center measurement.
